Special needs step brother needs help

Hello, my step brother Josh has refused to see a dentist in many years. We took this picture last night and we are VERY concerned. Just wondering what the general opinion is here and if it's too late for some of his teeth.
We also do not have dental coverage. He is unemployed and on SSI

Difficult to say to without an x-ray. You would need to see a dentist in person, so that the teeth can be evaluated, in order to discuss the best or cheapest treatment options.
