Detecting Lateral Portals of Exit

It appears that secondary canals are hard to image/find without being filled. In the endoblog example to the left, the lateral canal was basically determined indirectly by the large external lesion.

Does a cusp like in my case necessarily indicate a bifurcation/hidden lateral canal (tow images to the right)?
