Imaging Lateral Portals of Exit

It appears that secondary canals are hard to image without being filled. In the endoblog example, the lateral canal was basically determined indirectly by the large external lesion.

Does a cusp like in my case necessarily indication a bifurcation/hidden lateral canal?
