This a very, very large cavity. An x-ray would be necessary to determine if the decay is into the nerve. If the decay is into the nerve and the tooth is restorable a root canal, buildup, and crown would be the only way to save it. If there is so much decay that the tooth is not restorable you would need an extraction.
