Bad filling

Hello,

If a filling is done poorly and it develops a cavity underneath the filling just a few months after the original procedure, should the same dentist treat the tooth free of charge?

How can a patient go about requesting this?

How do you know a new cavity formed underneath the filling? In what country did you have this done? If the filling is, indeed, defective, then in the USA it would be standard practice to replace or repair it free of charge.
