Afraid I need this implant replaced because of how far it appears to be outside the lower arch. Opinions?

I’m very anxious that my lower-right second-molar implant may be positioned too far toward the cheek and possibly too far back.


I’ve had ongoing problems around it:


  • Constant food trapping
  • Years of gum irritation
  • A crown that feels narrow at the gumline and visibly tilted
  • No normal static contact
  • Concentrated angled contact during jaw movement(hits at an angle and hurts allll around the gums around implant crown and into the next crown on contact)
  • The implant crown is not centered beneath the opposing tooth

Several nearby crowns also seem to have been shaped around this implant, and the whole area now has abnormal contacts.


The current abutment and crown already appear to be compensating for the implant position. I’m worried that further customization would require an even more angled crown rather than correcting the underlying problem.


I understand a panoramic image is only two-dimensional, but based on what can reasonably be seen:


Does the implant appear so far outside the arch that implant replacement may need to be considered, or could the spacing and centering potentially be improved by replacing the crown and/or custom abutment?


I’m especially frightened about the possibility of implant removal and nerve injury.

To say if the implant is placed in or outside the arch, is not something that can be see on the x-ray provided. Do you perhaps have a photo that you can share, showing from the top as well as when you bite. This will provide a better idea of what the situation is in terms of placement.
