Permanent excruciating pain for over 10 days in #21 following the extraction of #46 [longitudinal crack in mesial root] four weeks ago. Periodontal Ligament stretched. Bite was adjusted and mandibular nightguard ordered.

Hope that's it...and that the pain will subside soon.
