Concerned about deep grooves in molars

Worried about LL7. I've obviously got very deep grooves in my molars (I've attached a photo of LR7, too, for comparison). The difference is that there's some sort of double ended, deeper, groove in the left but not the right? I've just looked in the mirror and noticed - the tooth has been aching last few days. Its not sensitive to heat, or to tap. The gum around it does bleed when brushing.


Nothing was flagged at my checkup in June.


I know there's a lot of black/brown staining, I have tried and failed for years to get it off myself (water flosser and electric toothbrush). I regularly see a hygienist to clear it, my next appointment is just after Christmas.

I would suggest taking an x-ray of both these teeth. It looks like there might be some decay present and then these teeth would need fillings.
Does your oral hygienist take x-rays when you go for your check-ups?

Yes, x-ray was taken 6 months ago; LL6 was filled, LL7 was ok

LR7 does look like there might be some decay in my opinion. I would suggest having another look at the buccal surface of this tooth. Also check the 'bite' or occlusion on this tooth.

Should I be concerned that LL7 has that sort of groove that breaks off into two forks? LR7 does not.

LR7 is very questionable, and I would probably drill. Picking up occlusal caries with bitewing xrays is only about 60% accurate, so sometimes I have to make a judgement call. I'd rather overdrill occasionally than miss in these situations. The Diagnodent/Diagnocam can sometimes help make the decision if your dentist has one..
