Would this require a cbct scan?
My surgeon thinks no, but I'm nervous

I usually look at the tooth's proximity to the inferior alveolar nerve. In your case, there is sufficient space between the nerve and the roots of the molar.

If this was done a few years ago is another X ray needed or will the nerve not move ? Thanks!

The nerve won't move, but there is always a chance that the tooth could have moved a bit. I would suggest taking another panoramic x-ray, just to confirm that it looks similar to this image. Not necessary for a CBCT.
