A tooth was filled (cavity) in December. In February it turned out to need a root canal. Did the dentist miss the root canal the first time or did the filling that wasn’t done properly cause the root canal and subsequent infection? Or is it normal to have this happen?

It could be either/or. You;d need to have x rays from before and after so that the dentists here can analyse.
