I am 5 days post molar extraction, everything seemed to be healing ok (although still in a lot of pain in surrounding teeth and swollen jaw) however today I have noticed a large white piece of bone/cartilage? that seems to be rising out of the wound. Not sure if this is bone spicule? It doesn't feel hard (although I am trying not to touch it). Really do not want to have to go back in as it was a traumatic extraction :( best image I could get attached, any advice really appreciated!

If it was a surgical or traumatic extraction, there is a chance that small pieces of bone might extrude from the socket, throughout the healing process and even afterwards, if the gum is fully healed. This is normal and a lot of times it is the interradicular bone that was present inbetween the roots of the teeth.
If the pain persists however and does not decrease, and is accompanied by a bad taste and smell, there is also a chance that you might have a dry socket, which will require a follow-up visit ( usually less traumatic than the extraction).

It sounds like you’re describing something that could very well be a bone spicule, which sometimes happens after extractions when tiny fragments of bone work their way out of the healing socket. It can definitely be uncomfortable and even a bit alarming, but usually it’s not dangerous. Often, your dentist can gently remove it in a quick visit, which can actually relieve a lot of the irritation and help the area heal more comfortably. Swelling and soreness in the surrounding teeth and jaw are normal at this stage, but if the pain gets worse, you notice a bad taste, or there’s persistent swelling, it’s important to get checked sooner rather than later. Even though the extraction was tough, a short follow-up with your dentist can make a big difference in your comfort and healing.
