Implant removal and continuing sinus drainage #14

I am a 67 year old male. I had an implant placed in my#14 on 3/1/23. The implant went into my maxillary sinus. I started having sinus drainage on that side right away and went on 2 rounds of antibiotics. Infection was real bad and I was snorting back a lot of yellow discharge from the left side. 9/27/23 a CT scan revealed infection around #14 in the maxillary sinus. On 2/24/24 implant #14 removed. Infection resolved after removal. 6/24/24 implant replaced and went into the maxillary sinus again. Infection started returning after replacement of implant. I saw an oral surgeon who said I had bone loss and inflammation and the implant was removed on 7/17/2025. Now I am still having sinus drainage and when I do a sinus rinse I can feel a sensation in the extraction site that I believe is due to the rinse solution in the maxillary sinus. Is it too soon to expect the drainage to be resolved after about 5 weeks? My ENT recommends a nasal CT but I had two done in 2023 and really don't want that much radiation.

After the sinus was opened, was the sinus closed surgically by an oral surgeon, and packed with bone?

Thank you for the reply Dr M. When the first failed implant was removed it was packed with bone. When the replacement second implant went in the periodontist drilled thru the sinus membrane and there was air between the mouth and sinus and I am not sure if bone was placed then but the implant did go in and was into the sinus . That second implant failed and showed inflammation and bone loss. It was eventually removed by an oral surgeon in July. I was asleep for this procedure but I do know he did take some of my blood and do something with it to put in there and also did a buccal ? flap - it was my cheek pulled down to cover the hole. Now when I do a sinus rinse I can feel a sensation in the site where the implant was removed. I won't say pain but a noticeable sensation when the salt water rinse first goes in. I usually do 3 rinses with one bottle of Neilmed solution and the sensation is only on the first rinse.

That ghost sensation is normal after a sinus opening was repaired. It sounded like he did a flap to close the opening. You have to consider that perhaps you are not a candidate for an implant in that specific area and maybe you need to consider alternative options, such as a dental bridge or partial denture.

Thank you Dr. M.
