I have lower jaw pain under ear and neck pain for 5 months now. Went to different doctors. Dentist told me that upper wisdom tooth and fracture in lower first molar might be the reason for my pain. I extracted upper wisdom tooth. I went to Endodontist to check the fracture on lower first molar. The Endo couldn't see any fracture in the root and he didn't take any cbct scan. He just took x-rays and did some diagnosis. He told me that I have cold and bite sensitivity on that tooth and that needs root canal. I went back to dentist to ask if it's jaw issues or tooth issue. He still suspect it's from the tooth fracture and want me to extract and implant. I went back to Endo to try root canal. The Endo did do the root canal 3 weeks back. He couldn't spot any tooth fracture during the procedure. He did root canal and I still have the same symptoms. I went back to dentist to do crown.

He is not ready to do crown until I get the confirmation from the Endo that tooth is not culprit for my pain in jaw and neck and also no root fracture. I didn't go back yet for a follow-up to the Endo to ask again of any fracture present in the root and he missed to spot it?

Meanwhile, my dentist sent me to a tmj specialist to check for tmj and eagles syndrome. I went to tmj specialist this week. He did some tests and said mine looks like jaw joint issue because of muscle tightness and myofacial pain.

During the test, he gave me a cotton roll to chew on right side to see if the chewing cause more pain. So he gave me the roll and I chewed on root canal tooth hard. Will that cause damage? I told him earlier that no crown placed. He is not bothered about it and was focusing on tests. I am also in a thought to get some diagnosis atleast by now. I did chew for few seconds the cotton roll on the root canal tooth. Will that cause any more fracture? Is it normal to chew on root canal tooth after 3 weeks even if it's not crowned?

Please let me know your thoughts. Attaching the x-ray taken before root canal. The pan xray taken this week

Hello,

It is really difficult to tell exactly what causes you to experience the pain unless we do a thorough examination for you. Gentle chewing on the cotton roll will be fine. But try not to eat anything hard on the root canal treated tooth even it is crowned, because a root treated tooth is weaker than your natural tooth due to various reasons. Try to postpone treatments if the exact cause of pain is not confirmed yet, because it could be the TMJ, it could be the jaw, or it could be another tooth etc and treating the one tooth (that you think it might cause the issue but actually it is not) will not solve the problem.

Hope it helps.

Thank you @A_s. Appreciate it.
If you mean try to postpone the treatment, which treatment, which one did you mean? Getting a crown or tmj treatment?
I am planning to get a crown done because it's been root canaled for more than one and a half month. Dentist dont want me to extract that tooth for now. He said try crown and see if it helps and rule out other things. Even after ruling out other things, pain persists, he said we can think about extraction. Last month he was very sure and want me to extract and implant.he talk to the Endodontist and he convinced now to try crown. I am yet to get tmj treatment. The specialist want me to finish the crown work before his treatment. It's mouth appliance for day and night and it costs 4k$. I am just postponing that. Have to rule out neuralgia and eagle syndrome.

If the pain is due to the tooth, then after endo, maybe after a few days, you should feel less painful/sensitive.

If there is pain/symptoms still persist after endo or worsen - pain persists for about 1 month for you? then it could be:
1) The endo treated tooth still needs to be watched until the symptoms are completely gone before you proceed with the crown. This can be due to a number of reasons: the endo treated tooth may have a really deep crack (then the tooth has a very poor prognosis and it may need to be extracted) or your tooth did not respond well to endo treatment yet or did not have time to heal up yet, so rushing into doing a crown will not help.
2) Or the pain is not coming from the tooth you did the endo on, it is due to another tooth or jaw or could be anything, so you need to find out exactly what causes the pain, and you don't have to rush for crowning the endo treated tooth yet.

But, the endo treated tooth should have a well-sealed restoration i.e. a white filling, or a silver filling to cover the tooth so the tooth is well protected.

You can also try crown if there are fewer symptoms than before you did the root canal treatment. But there is no guarantee that the crown may solve the problem.

The main thing is to find out exactly what causes the pain.

These are only the advises I can provide base on the information you posted on here. They may not be correct because I did not do a thorough examination for you. Please have a thorough talk with your general dentist.
