Periapical infection never really healed. Kinda forced obturation 140 days later. Pain, headache, and malaise picked up thereafter, culminating after 8 weeks, then got a bit milder. After 650 days of constant post-obturation pain, tooth was extracted and implant/osseus graft was inserted.

Question: why did the root canal fail from the getgo?

Was there a carious lesion? Did you have trauma to the tooth? The initial peri-apical does not reveal any peri-apical abscess formation? Did you have spontaneous pain or lingering pain with hot and cold stimuli?
