root canal mistakes?

I had a root canal procedure done on a lower-right molar that was having tooth ache pain a couple of weeks ago. I had extreme amounts of pain throughout the procedure. A couple of days after the permanent crown was installed, I experienced extreme pain and massive swelling the size of a golf ball in lower jaw and gum area from some kind of infection. I was given antibiotics and the swelling has gone down and pain is subsiding.
The dentist recommended seeing a "gum specialist" now to prevent further infection, but I am still confused about what caused the swelling and infection. The dentist did not mention anything about needing more treatment after the root canal and she would not explain what caused the swelling and infection. Also, I recall during the root canal procedure hearing the dentist mention to his assistant that there was hemorrhaging and part of the root canal procedure needed to be "started over again". Gingevectomy was also performed during root canal and during installment of permanent crown.
Could there have been some mistake made that resulted in complications and need for further treatment?? Now I am being told that I have rampant periodontal disease and that I may need a bone graft in my jaw or may need to have the tooth completely removed. I am confused and having difficulty making a decision about this because there was no mention of periodontal disease before the procedure and the dentist recommended root canal instead of pulling out tooth. Was the wrong procedure done? Was something missed; failure to kill infection? How should I procede?
