Hi,

I am a 30 year old male and was diagnosed with moderate to severe
periodontitis with probing depth from 6 to 8 mm at some places, I have
performed flap surgery with bone graft and emdogain treatment for upper
jaw, and are about to undergo the same thing for the lower jaw, the
upper jaw seems to recover pretty well and the pocket depth have all
been reduced to < 4 mm. I am undergoing a strict regime of three month
professional cleaning and uses interdental and electric brush after
every meal.

Question is, is periodontitis something that can be stopped all
together with no further bone loss, or is teeth loss an inevitability
leading to the need of dental implants down the road ?

Most times, yes.

Not necessarily. Though there is something called 'refractory'
periodontitis, that just means the patient doesn't respond to treatment.
Rather rare IME, but it does happen.

Please explain explicitly if possible, what is flap surgery with bone graft
and what is endogain treatment. How does this differ from deep cleaning
and scaling. What is the cost for such treatment.

Thanks, knowing that it is still something I can control and prevent
from deteriorating even at this advanced stage is a motivation to keep
my teeth clean.

Here is how I understand it: flap surgery, bone graft, rooting and
scaling and emdogain treatment are typically all done during one
surgical procedure, all are done to reduce gum pocket size, clean your
teeth in areas typically not reachable by you, treat the teeth to
reduce the possibility of more bacteria built up, and try to regenerate
some of the lost bone and supporting tissue. I think it costs about
1500 dollars for one quadrant of your teeth. Done properly it will
reduce your pocket size to a manageable depth for easier cleaning and
raise your bone level up to 2mm ? depending the type of your bone
defect.

Does that mean $6,000 for the entire mouth. I guess with teeth, first
its knowledge, then its money. Then its persistence, persistence, and
persistence and good diet with the foods that are less likely to cause
tartar. I think packaged white bread is one of the worst, except for
toast. That kinda includes the French bread. This is only speculation,
though.
