Is this billing unethical?

Cost of braces: $5000
My insurance pays: $2500
My cost: $2500

I have had the braces 2 years. They are still on my teeth and I have paid my $2500. One year into the braces I switched jobs and my insurance discontinued. I assumed insurance had paid their part, but they had only paid $450. I called the insurance company and it turns out that insurance was only contracted to pay $900 to the orthodontist in the first place. Ok, I say, I'll pay the $450 insurance had not yet paid. No, said the ortho, you now owe $2,050.

What is going on? It seems highly unethical to me.
